Return-to-Sport after ACL reconstruction

Poor functional performance 1 year after ACL reconstruction significantly raises the risk of early knee osteoarthritis progression. We test objectively (strength, jump, movement quality, LSI ≥ 90 %) before return-to-competition is cleared.

Current scoping reviews show that return-to-play cannot rely on single criteria. A multimodal approach is needed – clinical tests, isokinetic strength measurement, sprint diagnostics and performance criteria. We mirror this protocol 1:1 in the Performance Lab Landshut.

Jump tests & movement quality

Lower extremity · Rehab

Jumps and movement quality (Drop Jump, Single-Leg Hop Battery, LESS score) are central markers for safe return to sport. Strength values alone are not enough – neuromuscular control decides re-injury risk.

Lateral ankle sprain (LAS)

Ankle sprain · Return-to-Sport

Wagemans et al. show there is no uniform return-to-sport standard after LAS – although re-injury rates exceed 70 %. Our protocol combines Star Excursion Balance Test, hop tests and sport-specific loading.

Performance assessment

Entry diagnostics: isokinetics (strength & H/Q ratio), jump diagnostics, movement quality, mobility, asymmetry index. Hard numbers instead of guesses.

Manual & active therapy

Pain-free and full range of motion. Manual therapy per IFOMPT, shockwave for tendinopathies, targeted strength training to tonnage plan.

Reathletisation

Periodised strength training, plyometrics, sprint, change-of-direction. Building up to sport-specific peaks.

On-field rehab

Transition from rehab room to pitch: match simulation, sprint diagnostics, LSI re-test. Clearance only at ≥ 90 % bilaterally.

Return-to-competition

Written clearance with rehab report for doctor, coach, club. Including a re-injury prevention programme for the first 6 months.
